Continuation of immunosuppression vs. immunosuppression weaning in potential repeat kidney transplant candidates: a care management perspective

چکیده

Management of immunosuppression in patients with a failing or failed kidney transplant requires complete assessment their clinical condition. One the major considerations determining is whether not such an individual considered candidate for re-transplantation. Withdrawal re-transplant can result allosensitization and markedly reduce chances repeat transplant. In this review, we summarize effects reduction on HLA sensitization, discuss impacts these patients, explore protocols future directions. Risks chronic immunosuppression, medical management allograft, effect nephrectomy are covered elsewhere issue.

Weaning of immunosuppression in liver transplant recipients.

Immunosuppression has been sporadically discontinued by noncompliant liver allograft recipients for whom an additional 4 1/2 years of follow-up is provided. These anecdotal observations prompted a previously reported prospective drug withdrawal program in 59 liver recipients. This prospective series has been increased to 95 patients whose weaning was begun between June 1992 and March 1996, 8.4+...

BK Virus in Kidney Transplant Recipients: The Influence of Immunosuppression

The incidence of BK virus infection in kidney transplant recipients has increased over recent decades, coincident with the use of more potent immunosuppression. More importantly, posttransplant BK virus replication has emerged as an important cause of graft damage and subsequent graft loss. Immunosuppression has been accepted as a major risk for BK virus replication. However, the specific contr...

Immunosuppression minimization: current and future trends in transplant immunosuppression.

The past decade has witnessed unprecedented advances in renal transplantation propelled by novel and effective immunosuppression drugs. The introduction of mycophenolate mofetil (MMF), tacrolimus, cyclosporine microemulsion, sirolimus, a new generation of monoclonal antibodies (the anti–interleukin-2 receptor blockers, daclizumab and basiliximab), and the depleting polyclonal biologic Thymoglob...

Immunosuppression in the failing and failed transplant kidney: optimizing outcomes.

There is little data to guide clinicians on the optimal management of immunosuppression in patients whose kidney transplant has failed and who have returned to dialysis. Nor is there robust data on whether to perform a transplant nephrectomy. Finally, management of late stage chronic kidney disease, including deciding on dialysis initiation, modality and access planning, must occur simultaneous...

Immunosuppression in kidney transplantation

For most patients with chronic kidney failure, kidney transplantation has the greatest potential for restoring a healthy and productive life. The risk of acute rejection is the highest in the first months after transplantation (induction phase) and diminishes afterwards (maintenance phase). Immunosuppression should be at the highest level in the early period and reduced for long-term therapy. A...
